You understand that the information submitted with be treated with strictest confidence by Biggin Runways Running Club (and by those acting for and on behalf of Biggin Runways Running Club) for services that I may wish to engage in now and in the future. In submitting this form you confirm that you have read, understood and completed this form for yourself. Any questions you had have been answered to your satisfaction. You understand that you am responsible for monitoring yourself throughout the exercise programme and that if any unusual symptoms occur, you will cease participation and inform your instructor and your doctor. 

You will undertake to notify your instructor at once if there is any change in your condition. You are aware that physical activity can be hazardous and there is a risk involved. You acknowledge that you participate at your own risk and take full responsibility for your actions. You also agree to Biggin Runways using your information to contact you about club business.
